Your first visit will require extra time in order to complete your medical history, so please arrive 15 minutes earlier than your scheduled appointment. If you have dental insurance, please bring your insurance information to your visit. When you arrive, check in at the reception desk and present your insurance information to the receptionist.

Take your child to the dentist. Patients under 18 years of age will need the signature of a parent or guardian on required forms.

Make a list of questions to ask our office, so you don't forget anything on the day of your appointment. This includes any concerns you have, or oral problems you've been experiencing.
