It is important for us to have any records you may have from a prior dentist, especially x-rays if they are less than 4 years old. If you have an involved Medical History or are taking a lot of medicines, please make sure you have written down all the exact names and dosages for each.

Save time at your visit by completing your registration forms at home. Simply print the Patient Forms, fill in the requested information, and bring them to your appointment. You can always complete the forms in our office too.

Take your child to the dentist. Don't wait until there's a problem. Have your child's teeth looked at every six months when they're young. Promoting good oral hygiene at an early age is one big way to get your child used to the importance of taking good care of their teeth. Don't forget to bring records of your child's complete medical history.
